Patient Participation Groups

Dear Patient,

We would like to know how we can improve our service to you and how you perceive our surgery and staff.  

To help us with this, we have set up a patient representation group so that you can have your say.

In the past we have struggled to run patient participation groups due to lack of interest so we decided to ask patients whether they would be happy to give us feedback on line. 

We currently have a wide range of members aged between 18 and 84 years. Our patient reference group also helps us decide what issues to look at in our patient survey. If you would like to join our patient reference group please complete the form below and return it to the surgery. Our practice manager keeps a register of all members and we never give your information or emails out to anyone else. From time to time we will for your feedback on issues such as the services we provide or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn't take too much of your time.

We aim to gather a group of patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
