When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
Practice Area
As a guide our practice area covers mainly North Shields, extending west to Churchill Street Wallsend, east to the coast, south to the Tyne Tunnel, and north to Rake Lane Road, Norham Road North, and the A1058 Coast Road.

Named Accountable GP
All patients registered with the practice are allocated a named, accountable GP who is responsible for overseeing their overall care. Patients aged 75 and over will also be assigned a named GP to help coordinate their care where needed.
You have the right to express a preference for which GP you would like to be assigned to, and we will do our best to accommodate this where possible.
However, this may not always be possible due to factors such as GP availability, annual leave, or sickness.
If you would like to request a particular GP as your named GP, please speak to reception or contact the practice.
